    Bliv Telehealth Informed Consent

    Effective date: October 15, 2026

Please read this consent carefully before you request care. It explains how Metabody Medical P.A. and its affiliated professional entities listed below (together, the “Medical Groups,” “we,” “us”) provide medical care through the Bliv platform, what telehealth can and cannot do, and your rights as a patient. When you check the box at the end, you are telling us that you have read it, understand it, and agree to receive care by telehealth on these terms.

    1. Who will be treating you

Your care is provided by physicians, nurse practitioners, physician assistants, and other clinicians who are employed or engaged by the Medical Groups — independent, physician-owned medical practices — and who are licensed in the state where you are located when you receive care. The Medical Group responsible for your care depends on the state where you are located:

    Entity nameStates served
Metabody Medical P.A.All states except as stated below
Metabody Medical Associates of California PCCalifornia
Metabody Medical Northeast P.C.New Jersey and New York
    Metabody Medical of Kansas P.A.Kansas
    Metabody Medical of Colorado P.C.Colorado
    Metabody Medical of Texas PLLCTexas

You will see the name and credentials of the clinician responsible for your care in the Bliv app, and you may ask for more information about any clinician’s licensure at any time. Metabody Services, LLC operates the Bliv website, app, and technology, including the AI Assistant, and provides administrative support to the Medical Groups; it is not a medical provider and does not direct your clinical care.

    2. What telehealth is

Telehealth means receiving health care from a clinician who is not in the same place as you, using secure electronic communication. Through Bliv this may include: completing a medical questionnaire and uploading information, photographs, or documents that a clinician reviews later (“asynchronous” or store-and-forward care); exchanging secure messages with your care team; sharing data from connected devices, wearables, and laboratory tests; and, when your clinician determines it is needed or when state law requires it, a live video or telephone visit (“synchronous” care). Your clinician decides which of these is appropriate for you. Some services may require an in-person examination or laboratory testing before care can begin or continue.

    3. Benefits and limitations

Benefits. Telehealth can give you faster access to licensed clinicians, care from home, ongoing monitoring between visits, and a written record of your interactions. Limitations. Your clinician cannot examine you physically and relies on the information, images, and data you provide. Because of this, telehealth may not be appropriate for every condition, a clinician may decide you need in-person care or additional testing, and there is a risk that a condition could be missed or that a delay in diagnosis or treatment could occur. Technology can fail — connections can drop, messages can be delayed, and devices can report inaccurate data — and you may need to repeat a step or move to a different form of care. Your clinician may decline to treat you, may decline to prescribe any particular medication, and may stop treatment if, in the clinician’s professional judgment, telehealth is not appropriate or a medication is not right for you. Requesting care through Bliv is not a guarantee that you will receive a prescription or any particular treatment, and no result is guaranteed.

    4. Alternatives

You are free to seek care in person from your own physician, an urgent care clinic, a hospital, or any other provider instead of, or in addition to, care through Bliv. Telehealth through Bliv is meant to complement, not replace, a relationship with a primary care provider. We encourage you to share information about your care with your other providers, and we will provide your records to them at your request.

    5. Emergencies

TELEHEALTH THROUGH Bliv IS NOT FOR EMERGENCIES. IF YOU THINK YOU ARE HAVING A MEDICAL EMERGENCY — INCLUDING CHEST PAIN, DIFFICULTY BREATHING, SEVERE ALLERGIC REACTION, SEVERE ABDOMINAL PAIN, FAINTING, OR THOUGHTS OF HARMING YOURSELF — CALL 911 OR GO TO THE NEAREST EMERGENCY ROOM. IF YOU ARE IN CRISIS, CALL OR TEXT 988. MESSAGES SENT THROUGH THE APP ARE NOT MONITORED IN REAL TIME AND MAY NOT BE READ IMMEDIATELY.
If you experience a serious side effect from a medication, seek emergency care first and then tell your care team.

    6. Your responsibilities

Good care depends on accurate information. You agree to: give complete and truthful answers about your health history, current medications and supplements, allergies, pregnancy or breastfeeding status, and symptoms, and to update your care team when anything changes; tell us the state where you are physically located at the time of each interaction, and not use Bliv to obtain care while outside the United States or in a state where the Medical Groups do not have licensed clinicians; take medications only as prescribed and read the information provided with them; not obtain the same or similar medication from multiple providers without telling each of them; and read messages from your care team and respond to requests for information or follow-up. Providing false information may result in care that is unsafe for you and may lead us to end your care.

    7. Prescriptions, pharmacies, and compounded medications

If your clinician prescribes a medication, the prescription is sent to a licensed U.S. pharmacy chosen by the Medical Groups, which dispenses it and ships it to the address you provide. Pharmacies are independent of the Medical Groups and Metabody Services, LLC. No controlled substances are prescribed through Bliv. Some medications may be compounded — prepared for you by a state-licensed compounding pharmacy pursuant to your individual prescription. You understand and acknowledge that: compounded medications are not FDA-approved, and the FDA does not verify their safety, effectiveness, or quality before they are dispensed; a compounded medication is not the same as, and is not a generic version of, any FDA-approved brand-name product, even if it contains the same active ingredient; your clinician will prescribe a compounded medication only when the clinician determines it is clinically appropriate for you and will discuss the reasons with you; and the name and contact information of the pharmacy that prepared your medication will appear on the label and in your order details. Read all information provided with your medication, store it as directed, and report any side effects to your care team.

    8. Use of artificial intelligence in your care

Your care team uses software tools provided by Metabody Services, LLC, including an AI Assistant that uses generative artificial intelligence, to help review your information, summarize it, flag items that may deserve attention, draft communications, and answer your questions. You are being informed that artificial intelligence is used in your care. The AI Assistant does not diagnose, treat, or prescribe, and it does not make clinical decisions; every clinical decision, including whether to prescribe any medication, is made by a licensed clinician, and content generated by AI tools for your care team is reviewed by a clinician before any clinical action is taken. Information the AI Assistant gives you directly is for informational purposes and is not medical advice; it can be incomplete or inaccurate, so confirm anything important with your clinician. You can reach a human member of your care team at any time through secure messaging in the Bliv app, by email at [email protected], or by phone at 888-859-2882; we respond within one business day. When you interact with the AI Assistant, the app will tell you that you are interacting with AI, and you can ask it at any time whether you are talking to a person.

    9. Privacy, confidentiality, and records

Your health information is protected health information under HIPAA and applicable state law. How we use and disclose it, and your rights over it, are described in our Notice of Privacy Practices, which you acknowledge receiving by checking the box below. Your information is stored in a secure electronic health record operated by Metabody Services, LLC on our behalf under a business associate agreement, and is shared with pharmacies, laboratories, and other providers only as needed for your care or as the Notice permits. Electronic communication carries some risk: no system is perfectly secure, and messages could in rare cases be intercepted or misdirected. Please use the secure messaging in the Bliv app, rather than ordinary email or text, for anything containing health information. You may request a copy of your records at any time through the app or by contacting the Privacy Officer: Matthew Hall, General Counsel and Privacy Officer, c/o Metabody Services, LLC, 2943 Parkway Blvd, Unit 374, West Valley City, UT 84119 · [email protected] · 888-859-2882. Video or telephone visits are not recorded unless you are told beforehand and agree.

    10. Identity verification, eligibility, and fees

You must be 18 or older to receive care through Bliv. To protect you, we may ask you to verify your identity — for example, with a government-issued photo ID or a photograph — before prescriptions or laboratory orders are issued. Clinical services are paid for through the Bliv platform on a self-pay basis; we do not bill insurance, Medicare, or Medicaid unless we tell you otherwise, and we make no representation about whether a health plan will reimburse you. Fees, and the refund terms that apply to clinical services and medications, are shown to you before you pay and are described in the Bliv Terms of Service.

    11. Your right to refuse or withdraw; complaints; state-specific information

You may refuse telehealth care, and you may withdraw this consent at any time, by telling your care team through the app or by contacting [email protected]. Withdrawing consent ends telehealth care through Bliv going forward; it does not affect care you have already received, your right to your records, or your ability to use the non-clinical features of the platform. Complaints. If you have a concern about your care, please tell us at [email protected] or 888-859-2882 so we can address it. You also have the right to contact the medical or nursing licensing board of the state where you were located when you received care; contact information for each board is available in the app and on request. State-specific information. Some states require additional disclosures or consents for telehealth. If you are located in one of those states, the additional terms in the state addendum shown to you during onboarding apply to you and are part of this consent.

    12. Consent

By agreeing to provide consent to receive telehealth services from the Medical Groups and checking the corresponding opt-in checkbox within our Services you confirm that you are at least 18 years old and are located in the United States; that you have read and understand this Telehealth Informed Consent, including the limitations of telehealth, the emergency instructions, the information about compounded medications, and the use of artificial intelligence in your care; that you have had the opportunity to ask questions; that you consent to receive health care services from the Medical Groups by telehealth as described here; and that you have received the Medical Groups’ Notice of Privacy Practices.
Your consent is recorded with the date, time, and the effective date of this document and kept in your medical record. You may request a copy at any time. Questions before you consent? Contact secure messaging in the Bliv app, by email at [email protected], or by phone at 888-859-2882; we respond within one business day.
